Abstract:
Patients and caregivers must take an active role and become Savvy if they want to get the health care they want and need. They must become as knowledgeable as possible about all aspects of their or a loved one’s health care. The Savvy Patient Toolkit provides key guidelines and questions to ask, with over 40 forms to aid users in compiling their ongoing medical information for fingertip access and using that information in the right way, at the right time to help health care providers help them gain better health care and experience fewer errors.
